What is CPR?

First things first, let’s decode this mysterious acronym. CPR stands for Cardiopulmonary Resuscitation. I know, bit of a tongue-twister, right? Let’s break it down:

  • Cardio = Heart
  • Pulmonary = Lungs
  • Resuscitation = Reviving or bringing back to life

In a nutshell, CPR is a life-saving technique we use when someone’s breathing or heartbeat has stopped. Think of it as being a human life-support machine until the professionals arrive. Pretty amazing, if you ask me!

How Does CPR Work?

Now, you might be scratching your head and wondering, “How on earth does pushing on someone’s chest keep them alive?” Great question! Let me paint you a picture.

Imagine the body is like a busy city, and blood is the supply truck delivering essential goods (oxygen) to all the buildings (organs). The heart is the central depot, pumping these trucks out to the city. When the heart stops (cardiac arrest), it’s like the depot shutting down – no more supplies are being sent out.

This is where you and CPR come in. You become the emergency delivery service. By doing chest compressions, you’re manually pumping that central depot, forcing those supply trucks (blood) to keep moving around the city (body). You’re keeping those vital organs, especially the brain, supplied with the oxygen they need to survive.

If you’re doing full CPR with rescue breaths, think of those as topping up the oxygen in the blood – like refilling the supply trucks before sending them out again.

By keeping this emergency supply chain going, you’re buying time. You’re helping to prevent brain damage and keeping other organs alive until professional help arrives or the person’s heart starts beating on its own again.

CPR in the DRSABCD Approach

Now, CPR doesn’t exist in isolation. It’s part of a broader approach to emergency response that we use here in Australia called DRSABCD. Let’s break it down:

  • D – Danger: Check for any danger to yourself, bystanders, or the casualty.
  • R – Response: Check if the person is responsive.
  • S – Send for help: Call 000 or ask someone else to.
  • A – Airway: Check if the airway is clear.
  • B – Breathing: Look, listen, and feel for normal breathing.
  • C – CPR: Start CPR if the person is not breathing normally.
  • DDefibrillation: Use an AED (Automated External Defibrillator) if available.

CPR comes into play at the ‘C’ step, but only if the person is unresponsive and not breathing normally. It’s crucial to follow these steps in order – you wouldn’t want to start CPR on someone who’s just taking a really deep power nap, would you?  

The Crucial Breathing Check

Before we jump into CPR, let’s talk about that critical breathing check. This is where you’ll be channeling your inner detective, using three key senses: look, listen, and feel.

Here’s how you do it:

  1. Look: Tilt their head back gently and lift the chin. This opens up the airway. Now, watch the chest. Is it rising and falling regularly?
  2. Listen: Get your ear close to their mouth and nose. Can you hear the sound of breathing? Normal breathing is quiet and regular, not gasping or gurgling.
  3. Feel: Keep your ear near their mouth and nose. Can you feel warm air on your cheek from their breath?

Do this for no more than 10 seconds. If there’s no normal breathing or you’re not sure, it’s time to start CPR.

Remember, gasping is not normal breathing. If you see occasional gasps, that’s a sign CPR is needed.

This check is crucial because it’s the deciding factor for whether you start CPR. We only do CPR on people who are unresponsive and not breathing normally. If they’re breathing normally, we put them in the recovery position and monitor them instead.

How to Do CPR: A Step-by-Step Guide

Alright, now we’re getting to the heart of the matter (pun absolutely intended). Here’s your detailed guide on how to perform CPR:

  1. Check the scene: Before you channel your inner superhero, make sure it’s safe to approach. We don’t need two casualties!
  2. Check for responsiveness: Give them a gentle shake and shout, “Hey mate, are you okay?” (Or whatever comes naturally – no need for formalities in an emergency!)
  3. Call for help: If they’re unresponsive, it’s time to call in the cavalry. Dial 000 or ask someone else to. If you’re alone, make the call first, then start CPR.
  4. Check for breathing: Remember our look, listen, and feel technique? Use it here for no more than 10 seconds.
  5. Start chest compressions: If there’s no normal breathing, it’s go time.
    • Kneel beside the person’s chest.
    • Place the heel of one hand on the center of the chest, right between the nipples.
    • Place your other hand on top and interlace your fingers.
    • Position your shoulders directly over your hands, keep your arms straight.
    • Push hard and fast – aim for a depth of at least 5 cm (2 inches) at a rate of 100-120 compressions per minute. 

      Why so deep and fast? We’re trying to mimic the heart’s normal function. This depth and speed are what it takes to circulate blood effectively.

  6. Give rescue breaths:
    • After 30 compressions, it’s time for two breaths.
    • Tilt the head back and lift the chin to open the airway.
    • Pinch the nose shut.

    • Blow steadily for about 1 second, watching for chest rise.
    • Give a second breath, then return to compressions.

  7. Continue CPR: Keep this 30:2 rhythm going until help arrives, the person starts breathing normally, or you’re too exhausted to continue.

Remember, your position is crucial. Keeping your arms straight and shoulders over your hands allows you to use your body weight for compressions. This helps you maintain effective compressions without tiring as quickly. Trust me, CPR is a workout!

CPR for Children and Infants

Now, you might be wondering, “What if it’s not an adult? What if it’s a child or a baby?” Great question! The principles are similar, but there are some key differences:

For Children (1 year to puberty):

  • Use one or two hands for compressions, whatever allows you to achieve the proper depth
  • Compress about 5 cm deep
  • Use the same 30:2 ratio of compressions to breaths
  • The speed remains the same at 100-120 compressions per minute

For Infants (under 1 year):

  • Use two fingers for compressions, placed just below the nipple line
  • Compress about 4 cm deep
  • Still use the 30:2 ratio
  • Keep the same speed of 100-120 compressions per minute

The main differences are in hand placement and compression depth. We use less force for smaller bodies, but the idea is the same – we’re trying to circulate blood effectively.
